The CPG Machine: Building an organization that fosters success
The implementation of Clinical Practice Guidelines is an organizational challenge not a scientific one. It is a challenge that the key players are not well versed in or well equipped for. I advance a philosophy and a framework for the successful organization of the SGH team to successfully implement multiple CPGs and have a vibrant self-sustaining program around each one. The program focuses on getting each member of the group to contribute from their expertise and not be frustrated with obstacles. A worksheet serves as the main planning, communicating and mnemonic device for assisting all the members of the team to collaborate effectively. An accompanying essay/instruction sheet guides the team through the simple and effective establishment of systemic change that can lead to a "machine" in the organization that continues to produce standardized evidence based interventions despite the movement of individuals in and out of the process.

This program is fairly simple but it is a game-changer. While it is virtually without cost. Some intitial training by someone who has the "vision" would be necessary much of the time on start up. Once the program is running it will self sustain with minimal inputs, despite changes in personnel and changes in medical recommendations.

excellent work! I prefer the fractal of gears that interface to help the machine run.
www.wellnessweavers.com is a community-based interfaith and interagency vision of such a machine that also includes the VA. Indeed the first "test pilot" arrangement was made with a homeless Vet who now is employed in the community and buying his own home and off of public assistance.
Because all those agencies that I tried to engage in my concept 3 years ago looked at me like I was from outer space and sighted "no money" and "too complicated", I just had to do what I can, when I could, how I could, and on very little money...still doing it and making great headway.
